Motor Car Claim Procedures

  •  In Case of OD or Theft of Accessories
    • Report the loss to the nearest Police Station.
    • Take your vehicle to any of insurer's accredited motor shop for estimate.
    •  Submit the following requirements:
      • Copy of Driver's license & Official Receipt
      • Copy of Certificate of Registration & Official Receipt
      • Original Police Report
      • Duly Notarized Affidavit
      • Repair Estimate
      • Pictures showing the damaged parts
      • Stencil of vehicle's motor & chassis number
    • Repair will be authorized by a Letter of Authority.

 

  • For Auto Personal Accident
    • Submit the following requirements within six (6) months from the date of accident.
    • Requirements:
      • Police Report
      • Copy of Driver's license & Official Receipt
      • Copy of Certificate of Registration & Official Receipt
      • Original Medical Certificate
      • Original Official Receipts

 

  • For Carnap
    • Report immediately to Phil. Fire & Marine Insurance Corporation's Claims Manager, Atty. Malou F. Santiago.
    • Submit  The following documents:
      • Copy of Driver's license & Official Receipt
      • Copy of Certificate of Registration & Official Receipt
      • Original Police Report
      • Original Alarm Sheet
      • Original Complaint Sheet
      • Certificate of Non-Recovery (to be obtained after 90 days  from the date of loss as required)

 

  • For Third Party Property Damage
    • Report to the nearest Police Officer.
    • Make a sketch and ask the other party to sign his agreement to the sketch.
    • Take down TP's vehicle, name & address of owner & driver, insurance company.
    • Take note of damages sustained by your car & the other vehicle.
    • Get pictures of damage to both your car & the other party. Note: Do not offer or promise to pay the other party. Payment or repair will be made after validation of claim.
    • If your received demand letters or calls from third party, please refer or call ENGR. MARIO T. ADOREMOS of Claims Departments.
    • Submit the following requirements:
      • Motor Vehicle
      • Copy of your & TP's Driver's license & Official Receipt
      • Copy of your & TP's Vehicle's CR & Official Receipt
      • Original Police Report
      • Duly Notarized Affidavit
      • Repair Estimate
      • Pictures of damaged parts to the TP vehicle
      • Stencil of motor & chassis number of TP vehicle
      • Original Certificate of No Claim from TP Insurance Company

 

  • For Third Party Death or Bodily Injury
    • For injured persons, get medical help immediately. If you think you are in danger of being harmed by reason of the accident, leave the place but report the accident to the nearest police station.
    • Do not offer or promise payment to other party.
    • Report the accident immediately to Philippine Fire & Marine Insurance Corporation Claims Department in order to assign an adjuster who will handle the claim.
    • Submit the following requirements:
      • Copy of Driver's license & Official Receipt
      • Copy of Certificate of Registration & Official Receipt
      • Original Police Report
      • Duly Notarized Affidavit
      • Original Medical Certificate
      • Original Medical Official Receipts
      • Authenticated Death Certificate
      • Authenticated Marriage Contract / Birth Certificate
      • Original Funeral & Burial Official Receipts
    • Adjuster will assist in completing the requirements
